瘟疫 (wēnyì) refers specifically to a plague or pestilence – a severe, widespread epidemic, often historical. It is more specific and serious in tone than 传染病 (chuánrǎnbìng), which is the general medical term for any infectious or contagious disease. 瘟疫 is used for devastating outbreaks like the Black Death.
