肺炎 (fèiyán) is the specific disease pneumonia, an infection that inflames the air sacs in the lungs. 炎症 (yánzhèng) is the general term for inflammation anywhere in the body, not a specific illness. So 肺炎 is a type of 炎症, but you wouldn't use 肺炎 to describe a sore throat.
