脓包 has two distinct uses. Literally it means 'pustule' — a small pus-filled blister on the skin. Colloquially it's a strong insult calling someone a 'good-for-nothing' or 'worthless loser', implying they're as useless as a sack of pus. The figurative sense is far more common in everyday speech.
