心脏 (xīn zàng) refers specifically to the physical heart as an organ in the body. Unlike 心 (xīn), which can mean both the organ and the emotional/mind 'heart', 心脏 is only used for the anatomical organ or metaphorically for the 'core' of something (e.g., the city's center).
